If your school has more than
one Captain/Lieutenant, make additional copies of the
original. This application and a $45.00 registration fee for
each Leader, which covers all expenses, should be sent to
the Safety Council of Northwest Ohio no later than September
2nd.

form, a “Health” form, “Medication Authorization” form,
“Photo Release” form, “Safety Council Release” form and a
recommended “Clothing & Equipment” list. In order to get on
the buses on Wednesday morning, it is required for you to
bring all of the forms, completed and signed. These forms
must be completely filled out with signature and brought
with the child in order to get on the buses September 8th.
Informational packets will be mailed in August for those
that reply.

attend, please call the Safety Council Office at (419)
662-7777 ext 231 to reserve your space. If payment not
received, the space will be forfeited.
